Periodontal Maintenance Plan

For patients with Periodontal Disease

15% off 4th Periodontal Maintenance appointment*

Dental Advantage Plan is not insurance. It’s a plan that includes 100% of your preventive care, plus benefits on other dental services we offer. It makes dental care more affordable for the uninsured and easier for patients to keep up the schedule of preventive care we recommend to maintain your oral health.

Terms and Limitations of the Plan

  • This is an annual plan. Plan begins the date the plan fee is paid and ends one year later.
  • Plan will renew every year unless you request to stop it.
  • Fee is due in full at registration and is non-refundable.
  • Plan in not transferable.
  • Services cannot be billed to ANY insurance, including Medicare.
  • All Products are excluded from plan.
  • Discount does not apply to any whitening services or products.
  • Rates are subject to change annually.
  • Care Credit cannot be used for payment of Dental Advantage Plan. Care credit can be used for restorative work but will result in 10% reduction of discount.
  • Use it or lose it. Services do not accumulate.
  • Plan is only good at Mishicot Dental; therefore if you are referred to a Specialist, discounts offered to plan participants will NOT apply.
  • *15% discount on additional dental work is offered if services are paid for in full, day of service.

Patient has existing treatment plan from past. If they sign up now, would they receive the discount?

Yes, as long as they are a plan participant they would qualify for the discounts.

 

New Patient to our office. Do I qualify for the plan?

Yes, as long as you do not have insurance.

 

Never had cleaning here and need Scaling and Root Planing. Can they sign up now?

Yes, they can sign up for Periodontal Maintenance Plan

 

Parent doesn’t want to sign child up yet. Are they required to? Can they sign them up anytime?

It is up to each individual family on who will be participating in the plan. They can sign additional children up at anytime and receive lesser fee for adding child to Adult plan.
